Aminu Mohammed,Larbi Koomson and Abdul Aziz Yusif got the goals for Kotoko before Felix Addo and Eric Agyei scored for Istanbul.
Kotoko, two-time losing finalist in the last three seasons of the FA Cup are aiming for a domestic double this season.
In another fixture, Feyenoord continued their unbeaten run this season in all competitions after defeating former Cup winners, New Edubiase 5-4 on penalties drawing goalless in regulation time.
The second-tier side, who have qualified for the Premier League play-offs thus progress to the quarter-final stage of the competition.
